Client snapshot

A multi-branch home care agency in Tampa was losing referrals between the hospital discharge call and the first day of care. Intake lived in inboxes. Clinical charts stayed in the agency's care system.

The challenge

Referral coordinators tracked new cases in email and a shared sheet. A referral could sit for a day before anyone called the family back. About 18% of accepted referrals never reached start of care, mostly because follow-up slipped, not because the agency declined the case. Staff were also pasting more patient detail into email than the agency wanted outside the chart.

What we built

Zoho CRM runs the referral pipeline only: source, branch, coordinator, insurance-check status, and start-of-care date. Clinical notes stay in the system of record. A short Zoho Creator form lets the intake desk log a referral without a shared spreadsheet. The coordinator gets the case immediately, and a task fires if the family has not been contacted within two hours. We kept fields to the minimum the coordinators need to move the referral, and access is limited by branch.

  • Referral pipeline only: source, branch, coordinator, insurance-check status, start-of-care date
  • Zoho Creator form for the intake desk, replacing the shared spreadsheet
  • Task if the family has not been contacted within two hours
  • Branch-level access so one office cannot browse another's referrals
  • No diagnosis notes, no clinical photos, and no chart documents in Zoho

How the project ran

  1. 1

Week 1 — what may live in Zoho

Intake leadership and the agency's compliance owner listed every field. Anything that belonged in the chart was struck off before a form was built.

  2. 2

    Weeks 2–3 — one queue

The Creator form and the CRM pipeline went live at the busiest branch first. Coordinators stopped copying referrals into email.

  3. 3

    Week 4 — the two-hour rule

A task fires if nobody has called the family. That rule is what moved follow-up from about 26 hours to about 2.

  4. 4

Week 5 — the other branches

The same stages and the same fields rolled out to the other three offices. Referrals that never reached start of care fell from 18% to 4% over the following month.

“Coordinators can see which referrals are waiting without opening five inboxes. The chart is still where the clinical record belongs.”
